The patients must drink 10 cups of water a day within 2 weeks of surgery.

Does this mean that the patients must drink 10 cups of water in the two weeks before and two weeks after surgery, or only in the two weeks after surgery?

This is a very poorly written sentence. It is unclear to me (as an English speaker) as to what it means. It can be taken any of several ways.

If I read this in a doctor's note, I would guess (based on my previous knowledge) that this means I should be drinking 10 cups of water for each of the 14 days preceding the surgery.

The answer could be quite variable, depending on the context.
