If a book is laid before me on the desk, and it is open, I seeing a certain page, can I use all these?:

-I am at/on/in page nine.

I would never say 'in' page 9. A certain passage might be in page 9, but the pages itself would not.

"on" is the normal neutral, WB. "at" does see uses as a more specialized, nuanced use.

I can't imagine any use for 'in' with page, book yes, but not page.

If a book has usage notes on its pages, then can I say?:

-In/On a usage note, it says...

1. I am not sure about the preposition.
2. Is it possible to say A USAGE NOTE? It doesn't sound right, because we very often say NOTES.

'In' means contained within certain text. 'on' could see use if someone had scribbled something on top/ over the USAGE NOTE

Yes, if it's a USAGE NOTE then call it that.
