The questionnaire is for a little research project I'm doing in sociology. All information you give is confidential, and there is only ten questions, so it wont take long.
If you could take a couple of minutes to fill it out, I would be very grateful.
Your wording of the questions are off. Like omitting the option for 0 on the how many times you asked your doctor about alt medicine. A really big error on whomever written up the survey.
I'm glad you said that, because that will just become part of the evaluation. This really isn't all that important, I just need to demonstrate that I can evaluate some research, in terms of validity, reliability etc.
