Help! Will PAY for Marlboro Answer Sheet

I am in the contest and unfortunately my cat tore up my answer sheet - here at the last hour!!!

I am in dire need of the marlboro answer sheet - the full thing with the 100 spaces and the fourth page where you write down the names and insert the photos in the pocket.

I will pay someone who has this and is not using it 40.00 + the overnight fee of 14.95 if someone would be so kind as to send me this.
