I think the first is better grammatically. I think the second is more common (at least around here). I don't think that it matters.
I would use the first (as long as she is still alive).

"She is a former Miss Universe" is correct. If you use "was," you're suggesting that she is no long a former Miss Universe. Was her title taken away?
