On the phone, should I say, "Hello, is this Louise?" or "Hello, is that Louise?"

Either will do, but only if you believe you might be talking to Louise.

Suppose that you are Louise, how do you answer the question(s) above?

Use "This." "That" implies that Louise is someone other than the person you're talking to.
