My mom has been defending my aunt who has not got the correct care since having strokes. She was supposed to be assisted 24/7 by extended family, but they sometimes left her alone overnight, all day, did not ensure she got food, took her medicine and one time while they were away and did not ask anyone to stay she fell outside and was in the heat for two hours because she was unable to get up. Another family member who is very close to the extended family and close to my mom said to her “if there is a rift”. Does this mean she is saying to my mom that she would chose the extended family? My mom is very devastated.

Who knows? Call the cops; this is elder abuse.

You are asking people to comment on a sentence fragment.
On its own it could mean anything.
It could be:
If there is a rift we will try to redolve it.
If there is a rift ws will never speak again.
Or loads of other permutations.
