"How, during the height of Franco's dictatorship, do you make a film this scathing of Spain's ruling elite - bored, cynical people who consider anyone not of their class as anonymous and replaceable (the cyclist they kill never gets a name or even a face)."


The first half of the sentence is a question, the second half isn't, so a question mark at the end looks strange.
I could split it in two, but then the second half is an incomplete sentence without a verb.

Placing the question mark at the end is considered correct, even though it looks strange. Turning it into two sentences might be better:

How, during the height of Franco's dictatorship, do you make a film this scathing of Spain's ruling elite? The film shows bored, cynical people who consider anyone not of their class as anonymous and replaceable (the cyclist they kill never gets a name or even a face).

How .... and any form of is would require a question mark.
