It's a bit wordy. You could say: Please complete the attached form. If you no longer handle these, please forward it to the person who does.
I don't know why you'd assume they may no longer be completing these forms or that they wouldn't automatically either forward it to who does or let you know they're no longer involved; therefore, I wouldn't even bother with the second sentence.
