Your Bank without signing that document to prevent selling of information.
Collects information you provide, sells it to private companies, organizations, etc, In which have mail-in-advertisements, to creating a profile ( like police profile or CIA/FBI/HLS/NSA profile or permanent record profile ) on you in which various agencies could act on them.
Honestly I do not have any facebook account in "my name". I never will.
