In doing so altogether / all together, we can achieve in reality what we do individually.
1. Should it be altogether or all together?
2. Should it be we can, in reality, achieve instead? If not, what is the correct phrasing?

All together
All together is used for doing things with other people.
Altogether is used for situations where something is done in its entirety.
I am unsure about the second part of your inquiry. Both seem to convey the same message.
