"The True Buddha, Nichiren Daishonin," implies that there is one true Buddha whose name is Nichiren Daishonin, whereas "true Buddha Nichiren Daishonin," implies that Nichiren Daishonin is one true Buddha. It leaves open the possibility that there are several true Buddhas.

But it's poor grammar. Are you saying his first name is True; his middle name is Buddha?
