Does "rattle" here mean "an instrument contrived to make a rattling sound, esp. a baby's toy filled with small pellets that rattle when shaken"?

Yes. The rattler in question is a baby toy/noisemaker.

But the image you've given is a teething ring for babies who are presently teething.
