I recommend the book, "Elements of grammar", to all learners of English.
1. Are the inverted commas required?
2. If yes, should the comma before “to” be inside the closing inverted commas instead of outside?

In general, inverted commas are used for the titles of articles in a publication, or for song titles. The titles of books are usually rendered in italics or are underlined (the convention of underlining goes back centuries, and was commonly used in hand-written documents). Whatever convention is used to indicate the book title, the commas as punctuation are not needed, and should not be used:
I recommend the book Elements of Grammar to all learners of English.
Or
I recommend the book Elements of Grammar to all learners of English.
Note that the word "Grammar" should be capitalized in that title.
