narrowed down/doctor in charge

A subsequent brain scan did not indicate any abnormalities; as such, the cause of this condition was narrowed to his heart. As my husband underwent an artery bypass surgery in 2003 at the Singapore General Hospital (SGH), his doctor in-charge at SGH was summoned to Tan Tock Seng Hospital to examine him.

1. Shouldn't it be 'narrowed down' instead?

2. Shouldn't it be doctor-in-charge or doctor in charge? (no hyphens) instead?

"Narrowed down" would be the common locution. "Doctor-in-charge" would be preferable; and although I cannot say it is a rule, in my experience the hyphens, both of them, are required.
