In a news paper report, I fond a sentence like "There were several leads that led us to this person". Is "that led us" phase is correct? Or the correct phase is "those let us"?

Yes. It is correct. 'That' is being used as a conjunction. See usage (4) below.

No. 'That' (conjunction) does not change to 'those' for plurals.
