Nichikan Shonin was born on August 7, 1665, in Joshu Maebashi (Gunma Prefecture).
1. Should there be a comma after '1665'?
2. Should 'Prefecture' be in uppercase?

Comma yes.
Prefecture should be upper case as it becomes part of Gunna Prefecture. Now, if it said he was born in some prefecture that isn't named, it would be lower case.
