I have/had been trying to conceive for many years, and underwent in vitro fertilisation (IVF) treatment/treatments as a result/as a final resort. Finally, in 2013, I became pregnant with my daughter - Enxin. Unfortunately, she was born premature at seven months.
1. Should it be have or had?
2. Should it be treatment or treatments?
3. Should it be as a result or as a final resort?

Why "unfortunately"? I was born at 6 months, and I'm still here.
You should make the tenses consistent. You are describing a series of events or situations that are all completed, so you should use the simple past tense.

"Have," Tang, means you're still trying
"Treatment" or "treatments" can be omitted entirely
"As a final resort" means you had tried other approaches
"She was born premature" leaves your Average Clod (me) wondering if she had survived
