If the academic title of a lecturer is Prof. Dr. John Smith, what would be the most appropriate way to address him in an e-mail?

No, I did not.

You address him or her as Dr. Smith.

Between doctor and professor, doctor wins out as a title. But professors don't always have doctorates. If you know for sure that your professor does not, then Dear Professor Smith is perfectly fine.
