What is the difference between "floor" and "ground"?

Normally, floor is inside a building or occupied structure. Ground is outside.
When I was young, (I am from England), the "floor" was what is underfoot indoors, and the "ground" was what is underfoot outside. However this difference is being eroded, and many UK people (particularly estuary speakers) refer to everything as the "floor", which annoys me, I have to admit. It is towards the lower end of the scale of registers.
