the names will be the same, the fingering may be different--a C chord is the same three notes, no matter what instrument you play it on. If you have a baritone uke, it's commonly tuned the same as the top four strings of a guitar DGBE (altho the D string is tuned an octave higher than it would be on a guitar), so you can just use the guitar chor diagram's top four strings. Soprano ukes are tuned differently. If you have guitar chord names, you can google to find the uke fingerings that they correspond to.
