Recently I read a very interesting book about not automatically separating Co-joined (aka Siamese) twins just so they would be like everyone else--and able to have private sex lives, albeit in some cases with reconstructed sex organs.

In this case, I think it might be wise to wait a bit to see which left arm had more function. Acting quickly for the sake of instant "normal" could be a big mistake.
