What the hell does that mean?
this must be a PRN med.
meaning.. take 2 once a day.
if that isnt enough, repeat that
max - 4 times a day.
a bit confusing.

Take 8 at once and let's see what happens.
It means that the person who wrote the instructions up needs to take a class on writing with clarity.
What kind of a pill is it? It sounds to me that normally a person would take two pills a day. If need be, he could take up to four doses (eight pills) max.
If you are unsure, it might be a good idea to check with your pharmacist, or even Google the medication, to see what is the reccommended safe dosage.
If it had said "Take 2 tabs QAM(every morning), then one Q4H (every 4 hours) prn (as Needed)", that would have made sense. As written, it doesn't make sense. Is it an analgesic (for pain) or anti-inflammatory? What kind of med is it?
