In Western cowboy movies, if someone spins a gun around their fingers to look cool, is there a risk of it accidentally firing?

In Hollywood films? Never.
In real life? All the ******* time.
Single-action revolvers have to have the hammer cocked so it is nearly impossible to accidentally fire when spinning.
Double-action revolvers need a fair amount of pressure to fire while spinning…so not impossible but somewhat unlikely.

Furthermore, simply remove all of the bullets eliminates all danger.
