Would this be grammatically correct. Paranoia; only real if it isn't. So smile! You're just paranoid! Or you're right Smile

No, a sentence makes sense on its own. That is also true for a sentence fragment separated by a semi colon.

Some of your sentences and sentence fragments make sense on their own.

“Paranoia.” “ So smile.” and “You’re just paranoid.” make sense on their own.

However, “Only real if it isn’t.” and “Or you’re right.” do not make sense on their own, they are dependent on what precedes them.

So no, it’s not grammatically correct.
