So what do you call an 18 year old kid who is Tall and thin and boyish (6'3" and 155 lbs) but is also athletic and could hold his own in a fistfight. Would he be a twink or something else?

You might call him a "strapping youth". I don't know exactly what that means, but it sounds about right.
