Irregular vaginal bleeding and a slight itch (inside)

Hi y'all!

I have been lightly bleeding (but enough to need to use a panty liner) since Thursday night (the 27th), but I'm not supposed to be on my period. I actually just finished my period one week ago (Sunday, the 23rd).
Anyways, I also have a really small itch on the inside of my vagina, but it's not intense.
No pain, I'm on the pill and I take it religiously, and also use condoms. What could this be??
