Which is more appropriate?
This is to certify that Name, born in City (Country) on Birth Date was awarded the degree of DOCTOR OF PHILOSOPHY In PhD Programme XXX by the University of XXX on Date XXX. His PhD dissertation is entitled: “Thesis Title XXX"
OR

Why do you need to know this? Are you creating your own degree certificates? Most or all universities will have standard wordings for degree documentation. Why do you need this?

The university does not issue English certificates and they give this as a declaration about the degree but there is a flexibility in the form based on your needs especially if you are going to use it abroad. I am asking about the correctness of the form in this post. Can you help?
I suggest that your first version, with a slight alteration may be sufficient:
This is to certify that Name, born in City (Country) on Birth Date was awarded the degree of DOCTOR OF PHILOSOPHY In the XXX PhD Programme by the University of XXX on Date XXX. His PhD dissertation is entitled: “Thesis Title XXX"

Most universities have relationships with other universities around the world for student exchange etc. If you represent a genuine university you should contact one of your English speaking partner universities and ask them.
