Where can I get a 5-way gearbox (like the flickr picture)

Is that one input and 4 driven outputs? The drive shaft is probably turning a bevel gear, which drives 4 pinion gears. I've never seen such a thing, and not at all sure it exists, in reality.

The usual source for such oddball stuff is w.w.grainger and Boston Gear. I don't think you're going to find such a thing, but that should be your starting point.
