My baby might have had gone through much / a lot of suffering as evidenced by the many surgeries that she had to endure.

1. Is 'have had gone' correct?
2. Should it be 'much' or 'a lot of'
3. Should it be 'had had to endure' instead?

1. It's correct but unnecessary. You could simply say, "my baby might have gone..." and keep the same meaning.

2. You can use either.

3. Be consistent with your tenses. "Have had gone" and "had had to endure,: or "have gone" and "had to endure."

The "had" is wrong. Otherwise I agree with infrablue.
