What does "whether in A and E" mean?

Context:

Bad communication is probably the biggest single cause of complaints and it is closely linked to the topic of honesty. Unlike previous generations, today’s doctors have had extensive instruction in communication skills but it is a feature of the system that it is often the greenest shoots who are the most exposed whether in A and E or as the house officer clerking in a patient. There is a world of difference between acting out how you would deal with a particular situation in a workshop and having to face the cold reality of telling a patient that he has cancer.
